Duties of this LEAP Job Posting:

Reporting to the Director of Nursing the Certified Clinical Diabetes Educator shall, in conjunction with an integrated Primary care team, deliver culturally safe, Diabetes care and case management to the member First Nation communities. The Certified Clinical Diabetes Educator will work in partnership with other members of the healthcare team to provide holistic care to the entire community population. The Certified Clinical Diabetes Educator will provide assessments, screening, healthy lifestyle support, education, and Diabetes management, with the ultimate goal of improving health outcomes and facilitating access to services.

Liaison with other Health Care Providers and Organizations

  • Consult with Clients; Primary Care physician /visiting Physicians and Nurse Practitioners on an ongoing basis
  • Work closely with other Health Care Providers in the community
  • Provide referrals to various community programs and supports
  • Provide referrals to outside agencies and supports as needed.
  • Act as a Patient advocate when needed.

Health Assessments

  • Perform a comprehensive and focused health assessment that includes health history, health status, risks and considers the psychosocial, emotional, cultural and spiritual dimensions of the client.
  • Understand social determinants of Health
  • Develop care plans.
  • Assess the needs for advanced care and medical directives
  • Complete and document a thorough medication assessment
  • Review laboratory and diagnostic reports for clients and flag potential concerns for visiting physician and Nurse Practitioners
  • Provide telephone and in-person care

Health Management and Therapeutic Interventions

  • Provide diabetes care for clients with a wide range of health issues
  • Initiate and/or contribute to development of a care plan
  • Encourage independence and self-care
  • Initiate consultations with other services and health care providers
  • Provide follow-up care and consultation for clients
  • Coordinate services and care with the client
  • Provide interventions as per medical directives or medical orders
  • Help resolve client concerns

Health Education

  • Assess health education requirements and readiness of the client, including current knowledge, literacy levels, social supports, learning preferences and other factors that may affect education
  • Acquire, develop and evaluate teaching materials and tools
  • Provide education related to healthy living, injury prevention, illness, care and treatments
  • Identify health trends and research that may affect clients

Health Promotion and Prevention of Illness

  • Screening and monitoring to prevent the onset of health issues and/or minimize complications
  • Provide healthy lifestyle education and support
  • Provide health education regarding health promotion and prevention

Program Evaluation and Reporting

  • Collect and compile data for reporting requirements; explore avenues to streamline reporting and ensure effective collection of necessary health indicators in alignment with the KCA member communities, KCA and both federal and provincial governments.
  • Participate in problem solving issues related to program design, delivery, evaluation and client-related issues.
  • Have a KCA performance appraisal done as per KCA policy
  • Participate in the Accreditation process, both at KCA and at the community level
  • Provide proper documentation as per the College of Nurses or College of Registered Dieticians

Qualifications for this LEAP Job Posting:
  • Certification as a Registered Nurse in the province of Ontario with the College of Nurses of Ontario in good standing and a member of RNAO with LAP, or A valid Registered Dietician license and in good standing with the College of Dietitians of Ontario
  • Experience teaching/counseling individuals and groups of people
  • Must have CDE designation
  • Experience working with individuals with Type 1 diabetes and insulin pumps, preferred
  • Understanding and ability to apply Clinical Practice Guidelines as well as other acts and legislation that govern practice
  • Experience in program planning and evaluation and performance management
  • Knowledge of Anishinaabe culture and the social determinants of health specific to First Nations populations
  • Ability to apply a bi-cultural lens to work activities
  • Experience and understanding of Indigenous cultural issues, the geographical realities and social conditions within northern remote Indigenous communities.
  • Strong computer skills
  • Strong written and verbal communication skills
  • Minimum 5 years’ experience preferred
  • Effective organizational and critical thinking/problem-solving skills
  • Must maintain ongoing professional education
  • Current CPR/AED with HCP and First aid certification
  • Valid Driver’s license

  • Must be willing to travel when required.
  • Ability to effectively manage time and priorities.
