Duties of this LEAP Job Posting:

KCA Primary Care Nurse Practitioners work to their full scope of practice within an interdisciplinary team to provide comprehensive health care to clients living in First Nations communities within the Treaty 3 Region. This position requires a highly motivated individual who can work independently with minimal supervision as well as work collaboratively with other members of the health care team and community. In addition to meeting all of the entry-to-practice competencies for Nurse Practitioners as outlined by the College of Nurses of Ontario, the ideal candidate will be confident in their knowledge base and skill set in order to diagnose, treat, and manage a broad range of episodic and chronic diseases that may present in a primary care setting. The Nurse Practitioner will work closely with the KCA nurses, community health staff and colleagues from other organizations who service the communities, so maintaining professional relationships and positive communication is essential. The Primary Care Nurse Practitioner will need to have an understanding and respect for Indigenous culture, traditions and beliefs and be willing to incorporate both traditional and Western medicines into client treatment plans. This role also requires a commitment to evidence-based practice, leadership, continuous education, and participation in quality improvement within the healthcare team.

• Perform comprehensive health assessments in community-based settings for clients of all ages and stages of health including acute/episodic care and chronic care
• Order and interpret diagnostic tests (including CT, MRI, Ultrasound, x-ray, laboratory investigations, etc), to diagnose health problems
• Clearly communicate a diagnosis and develop a treatment plan based on diagnostic interpretation and clinical findings
• Provide treatment and ongoing monitoring of acute and chronic health conditions that include comprehensive pharmacologic and non-pharmacologic therapies
• Provide referrals to other care providers, specialists and specialized clinics when necessary
• Collaborate with all members of both KCA’s primary care health team, service providers (physicians, nursing, dietitians, physiotherapy, pharmacy, emotional wellness) from other organizations and community health center/nursing station staff
• Conduct screening for early detection of cancers and chronic diseases (i.e., pap testing, FIT testing, mammography, diabetes screening) and provide preventative health teaching
• Maintain and protect client confidentiality at all time
• Conduct home visits in collaboration with home care nursing staff when clients are unable to present to the health center due to physical limitations/chronic health conditions
• Document client care within the electronic medical record in a timely manner as outlined by the College of Nurses of Ontario practice standards
• Respond to critical laboratory results or diagnostic findings ordered and provide timely follow-up to clients
• Provide cross-coverage to NP/MD colleagues within the health care team; colleagues will also offer cross-coverage so that appropriate vacation and time off without interruption is allotted

Qualifications for this LEAP Job Posting:

• Current certification with the College of Nurses of Ontario-Extended Class and a member in good standing
• Successful completion of Master of Nursing, Nurse Practitioner Program (Primary Care/Adult)
• At least 1 year of experience as a Primary Care Nurse Practitioner is an asset
• Minimum 3 years’ experience as a Registered Nurse with experience in primary care/community care settings working with clients of all ages
• Current BLS certification
• Proof of immunizations in accordance with organizational policy
• Valid driver’s license and ability to travel for work
• Current criminal records check
• Must be highly self-motivated with a demonstrated ability to work independently
• Must have excellent interpersonal and communication skills with a demonstrated ability to be an effective member of the primary care team
• Demonstrated critical thinking and problem-solving skills
• Knowledge and expertise on health issues across the lifespan. Specific expertise in the following areas would be considered an asset: chronic disease management, diabetes management, geriatric care, mental health and addictions, cancer screening, and women’s health
• Demonstrated cultural competency and experience working in First Nations communities
• Experience using PS Suite electronic medical records is an asset but not required
