Duties of this LEAP Job Posting: Reporting to the Justice Program Manager, the Social Cultural Wellness Worker shall deliver culturally safe, trauma informed, clinical case management mental health and addictions services within correctional facilities in Ontario. This position includes counselling with individuals and therapeutic groups, linking individuals to appropriate services and maintaining accurate client records. This program does group programming within correctional settings. This program will include the development and delivery of strength-based programming, and aftercare through relationship building with local service providers to be able to assist and support individuals in making a greater positive impact within the community. The Social and Cultural Wellness Worker will work to help deliver and improve the services offered to the relevant communities.
In Custody work
• Provide one on one counselling, information, and guidance to clients with regard to mental health and addictions within the Kenora District Jail.
• Provide counseling to clients on specific ways to recognize and control problem behaviours, and information, education, and support to clients to manage their own behaviour.
• Provide group programming at the Kenora District Jail with a focus on Mental Health and Addictions, using traditional teachings and the Seven Grandfather teachings as a guide.
• Monitor clients’ mental status and behavior for decline and take appropriate action.
• If requested by client, facilitate traditional healing services. Note: This could be an alternative to, or in conjunction with contemporary methods.
• Coordinate and facilitate individual and/or group counselling to clients. This could include sharing circles, therapeutic debriefing sessions, etc.
• Participate in inter-disciplinary case conferences.
• Keep accurate, timely client contact records in accordance with program requirements.
• All data entry to PS Suites should be completed on a weekly basis.
• Alert the Program Manager and/or Director of Services on all critical cases and situations.
• Other duties as assigned.
Post Custody Work
• Facilitate linkages to needed services within the broader service community for example: treatment centers, safety/stabilization beds.
• Develop networks and partnerships with community resources.
• Encourage and support clients in building their own social lives, independence, and support systems to reduce dependency on the Wellness Worker.
• Advocate for client and coordinate case management meetings with relevant partners.
• Facilitate access to Anishinaabe knowledge, traditions and cultural beliefs in the healing process (if appropriate).
• Provide group programming with a focus on Mental Health and Addictions, using traditional teachings and the Seven Grandfather teachings as a guide
Monitoring & Evaluation
• Maintain statistics required by the Ministry and the Program with regard to qualitative and quantitative aspects of the Program.
• Contribute to the evaluation of service with respect to its effectiveness in meeting the community needs and achieving our mandate.
