Duties of this LEAP Job Posting:

Reporting to the Director of Primary Care and serving the KCA Member First Nations of the Treaty 3 area, the Clinical Dietician will work in partnership with other members of the health care team to provide holistic care to the entire community population or will provide services and care to the entire community population. The Dietician will support community members and clients with healthy lifestyle supports, education, chronic disease management, assessments, and home care programming through the provision of Diabetes care with an ultimate goal of improving health outcomes and facilitating access to services

  • Respects traditional First Nation practice protocol(s) when engaging with KCA’s members and clients
  • Plan and provide nutritional health education and services to KCA clients/groups including high risk groups suffering from chronic diseases
  • Supports all program initiatives designed to strengthen services and program integrity; use service evaluation feedback to purposefully and continually strengthen services
  • Deliver nutritional services to all KCA communities
  • Provide nutrition counselling from client referrals
  • Complete client nutrition assessments and develop nutrition care plans to enhance general health
  • Screening and early identification to reduce secondary impacts, and assist clients with self- management by supporting Health Promoters in their prevention roles, and providing direct services to clients
  • Cultivate and enhance client and member relationships and utilize motivational techniques to influence patients to balance treatments or plans? with their lifestyle
  • Assists in menu planning to ensure clients’ nutritional needs and food preferences are met
  • Coordinate services and care with the client, develop nutrition curriculum, educational and reference
  • Ensures there is appropriate consent related to intervention or exchange of information and reviews this regularly- materials and seminars
  • Provide, on specific request, consultation to family physicians and other team members to assist with individual patient care; assessing the needs for advanced care and medical directives
  • Assess, evaluate, and provide nutritional care plans for clients and communicate the plans to the health care team
  • Provide nutrition coaching, within the health center and on accompanied home visits
  • Provide follow up care and consultation for clients; initiating consultations with other services and health care providers
  • Evaluates new nutritional products and equipment to meet client-specific needs
  • Incorporate a research informed approach toward the delivery of all services
  • Plan, evaluate and conduct nutrition education programs and develop educational materials for various audiences
  • Work closely with other members of the health care team to provide nutritional education as requested
  • Participates in client reviews team meetings as required to ensure effective communication necessary to achieve a consistent, strong client focus
  • Present on specific nutrition topics that are relevant to KCA clients and Communities (e.g., Chronic Disease Management Educational Programs such as Diabetes, Cardiovascular Disease, and Health Promotion Educational)
  • Provide nutrition guidance, label interpretation and consultation services to health professionals, individuals, dietetic interns, community groups,
  • Provide consultation and support to First Nations and community staff on pertinent developments in diabetes and related fields such as validated prevention approaches
  • Participate in diabetes committees and meetings in the communities, region and with outside agencies and government
  • Develop culturally appropriate strategies, programs and activities for promoting Type 2 diabetes awareness, prevention and management at community level
  • Assist community-based staff to assist in empowering individuals and groups to make healthy choices (nutrition, physical activity) to prevent and manage Type 2 diabetes
  • Develop and deliver culturally appropriate training and capacity building opportunities to meet identified training needs for community-based staff
  • Reports and documents all high risk or potentially high-risk situations according to policy
  • Develops a clear understanding of health or client record documentation standards according to specific programs and policies
  • Documents and maintains client files in a format consistent with program protocol and timelines
  • Content development (blogs, handouts, presentations, and social media content)
  • Responsible for evaluating effectiveness and efficiency of interventions to ensure outcomes that are positive and beneficial to clients
  • Additional duties and responsibilities may be assigned provided they are consistent with the qualifications identified and fall within the parameters of this role

Qualifications

  • Current registration with College of Dietitians of Ontario; certified (or eligible for certification as a Certified Diabetes Educator)
  • Bachelor’s degree in Foods and Nutrition or related degree and successful completion of an internship
  • Knowledge of Anishinaabe culture and the social determinants of health specific to First Nations populations
  • Demonstrated cultural competencies to fully understand how to direct and facilitate the development and implementation of diabetes strategic plan in a culturally appropriate way
  • Experience and demonstrated ability to work within a wholistic care model; able to work independently and be an effective team member
  • Willing to work to full scope including performing delegated acts through medical directives
  • Understanding and ability to apply Clinical Practice Guidelines as well as other acts and legislation that govern practice
  • Understanding social determinants of health specifically in First Nations
  • Ability to apply a bi-cultural lens to work activities
  • Strong conflict resolutions skills

  • Strong written and verbal communication skills
  • Knowledge and experience in Primary Health Care and Social determinates of health
  • Effective organizational and critical thinking/problem solving skills

  • Proof of immunization in compliance with policy requirements is mandatory
